Appointment Policy

Please call the office in advance to schedule an appointment. If you are unable to keep your appointment, we kindly request at least 48 hours’ notice so that we may accommodate another patient in need of care. While we make every effort to remain on schedule, delays may occasionally occur due to emergencies or hospital procedures. We appreciate your patience and understanding.
 

Appointment Guidelines

  • New Patients: Please arrive 30 minutes early to complete the registration process. If your appointment is scheduled at 8:00 a.m. or 1:00 p.m., you may arrive at your scheduled time.

What to Bring

  • Valid Driver’s License or State ID
  • Insurance Card
  • A list of current medications and any known allergies
  • Copies of X-rays, MRIs, CT scans, and any prior medical records
  • Physician referral form (if required by your insurance)
  • Worker’s compensation forms (if your visit is related to a work injury)

Co-Pays and Deductibles

Co-pays and any applicable deductibles are due at the time of service. These will be collected during check-in. Please verify your benefits with your insurance provider in advance, as fees may vary. For your convenience, we accept major credit cards and cash payments.
